CREP: Person-Centered Planning

The goal of this person-centered planning training is to increase the capability of NYC Health + Hospitals’ workforce to perform specific activities in anticipation of a decline in hospital demand and a rise in the need for home and community-based services. Person-centered planning is an individualized, ongoing problem-solving process that is developed and revised iteratively.  This increased scope of skills will enable these direct care workers to be effective care team members and will prepare new members to enter the workforce better prepared as part of an ongoing pipeline of workers. The workforce development focus areas presented will improve services inside the hospital and help improve the workforce’s ability to provide care outside of an institutionalized setting.
